Licensing and Private Labeling

You can use any of the existing HealthEngage applications for use in your marketing or promotional program, publication or for inclusion in product packaging. Private labeling of a ready-made HealthEngage product or a specially designed customized version can give you an instant differentiating factor and can drive the recognition of your product. A whole range of options is available to license HealthEngage applications:

• You can select to license any existing product(s) as is for distribution on a CD-ROM. We will support you through the entire process, including inclusion of additional promotional information or content on a CD and CD duplication and delivery. Your logo, colors, and content can be integrated within an existing application for a completely privately labeled look.
• You can select to license any existing product as is for distribution through web-based download. We can provide you with a HealthEngage download page or a page seamlessly branded for your organization and, if necessary, integrated with your own web site. If you would like to provide software for download for your end users, but would like to include paper of plastic license cards into your packaging or publication, we can print and deliver individual license cards to your publisher or packaging company.

A license for certain application and system use can be given to an organization desiring to purchase over 10 user licenses. A licensing fee will be calculated on a volume discount basis from the listed retail price. The discount depends on the number of licenses purchased. For any private labeling contracts an additional per user license private labeling fee will apply. For stand-alone desktop and handheld applications only a per user license needs to be purchased. For any wireless or web-based applications or enabled application components an additional server hosting fee will apply in cases when data and the Application Server are hosted with HealthEngage. Existing Application Changes & Modifications, New Application Assembly, Custom System Development

You have several options for customizing an existing or ordering a new application:

• You can select to customize an existing application. Changes can be as small as changing the content or format of some of the questions or major, such as removing or inserting whole modules/sections (i.e. food, medication, wellness, symptom board, Internet synchronization, etc.). Any licensing and private labeling option above can be used in conjunction with your custom project.
• You can select to create your own application from the building blocks available, integrating custom question sets, interactive response modules, selecting deployment platforms (i.e. web, desktops, handhelds, web-enabled phones), design elements, and final user distribution methods. These modules are already pre-built and require minimal amount of time for customization. New features can also be developed on request. • You can select to create your own HealthEngage module for use in any of the existing applications or in the application of your own design. The modules we design have ranged in functionality from those supporting direct input of data from medical devices and blending it with patient input to clinical trial recruitment modules with direct display and trial recruitment for clinical trials.

Health Manager Interface Licensing and Configuration

A health manager interface is available for any HealthEngage system applications (i.e. those with an Internet component, such as web-based or Internet-enabled desktop applications) and can be ordered as a part of the package by any organization interested in managing multiple users and their data. This application can access individual data for use by healthcare personnel in a healthcare office setting, or de-identified patient data for surveying and data mining purposes. Before deployment, the interface is configured for your particular use. For instance, a health manager interface can serve as a school nurse/student data management application, which allows to access health information on each individual student (previously collected through a HealthEngage application), add additional information to each record, get alerts when certain thresholds, such as student days absent, are triggered, and chart group information, and so on. Each school nurse determines what level of access, data management, and visualization is necessary and the manager interface is configured and implemented based on this specification.

Health Manager Interface application is licensed in connection with a purchase of at least 10 HealthEngage application user licenses. All configuration work is charged on the per project basis, based on the estimated project implementation timeline.
